Include in your request

1. Last name, first name, patronymic, and date of birth.
2. Contact details — phone number, mailing or registered address, and email address.
3. Additional information that may help us. For example: office address, transaction date, amount, etc.
4. Attach the required copies of documents.

Check before sending

• Contact details. Your details in the bank's records and in the request must match. You can check and change your email address and phone number in the app or the Uralsib Online web version on the “More” tab in the “My Details” section. We can provide a response if the addresses in the request and those previously provided to the bank match.
• Text of the request and attached documents. After sending the request, you will not be able to change its text or send additional documents.
