To receive insurance compensation, please contact one
of the authorized offices of Bank Uralsib and provide the following set of documents:
An application for payment of deposit compensation (indicating the details for making the payment in the form established by the Deposit Insurance Agency (DIA)). Bank employees will independently complete and print the compensation payment application for you.
1. Legal entity:
- An identity document of the person authorized to act on behalf of such legal entity without a power of attorney, information about whom is contained in the Unified State Register of Legal Entities, or of the legal entity's representative acting under a power of attorney.
We recommend having with you the organization's charter in its current version (with amendments, if they concern the powers of the sole executive body) and the decision on election/order on appointment of the current sole executive body to confirm the applicant's authority.
- The applicant may also be:
- a financial administrator, external administrator or insolvency administrator approved by the arbitrazh court, or their representatives;
- a person authorized to act without a power of attorney on behalf of the legal successor of a legal entity that has acquired the right of claim for the deposit through reorganization, and their representatives (upon presentation of documents confirming the representative's authority). When applying for compensation payment on behalf of the organization's legal successor, documents confirming the transfer of the right of claim for the deposit to the legal successor shall additionally be provided.
When applying for compensation payment through a representative acting under a power of attorney, a notarized power of attorney shall additionally be provided. It must include powers to represent the depositor's interests when receiving compensation or be of a general nature (general power of attorney), that is, contain powers to use and manage all property of the principal and exercise his or her rights and obligations, including representation before any third parties on all matters and handling any affairs on his or her behalf.
- If the representative acts under a power of attorney, a power of attorney must additionally be provided (notarized or general). The power of attorney must contain authority to represent the depositor's interests when applying for compensation payment and/or when receiving compensation, or the power of attorney must be general (contain powers to use and manage all property of the principal and exercise his or her rights and obligations, including representation before any third parties on all matters and handling any affairs on behalf of the represented person).
2. Individual entrepreneur:
- An identity document of the individual entrepreneur or the individual entrepreneur's representative.
- Information on the Taxpayer Identification Number (TIN) and OGRNIP (may be provided orally).
- A representative of an individual entrepreneur acting on behalf of the individual entrepreneur under a power of attorney shall additionally provide a notarized power of attorney confirming his or her right to apply on behalf of the individual entrepreneur with an application for compensation payment, or a general power of attorney containing powers to manage all property of the individual entrepreneur. It does not matter whether such power of attorney directly indicates the representative's right to receive compensation for the principal's deposits or not. A sample power of attorney is posted on the agency's website in the section “On Deposit Insurance/Document Templates/Document Forms/Powers of Attorney/Sample text of a power of attorney for representation regarding receipt of insurance compensation”.
- If an individual entrepreneur is in the bankruptcy stage of “debt restructuring”, it is necessary to provide an identity document, Taxpayer Identification Number (TIN) information, and the consent of the financial administrator.
- If an individual entrepreneur is in the bankruptcy stage of “sale of property”, the financial administrator applies to the bank for compensation payment with an identity document and the depositor's Taxpayer Identification Number (TIN) information, or the individual entrepreneur (depositor) applies directly with an identity document, Taxpayer Identification Number (TIN) information, and the court act “On exclusion from the debtor's bankruptcy estate of the right to compensation in whole or in part”.
- If the individual entrepreneur has lost the status of individual entrepreneur, a supporting document must be presented (for example, an extract from EGRIP). In such case, compensation payment shall be made by analogy with individuals.
